I would use b), but it doesn't make a great deal of sense to me as a sentence. Is that all the context?

I would use b), although more context is needed since this can mean both 'payable from' and 'payable to': if you 'pay a premium for an account', you pay for the privilege of holding that account with the relevant financial institution; whereas if a premium is 'for the account of Mr X', it means it is payable into Mr X's account - I think this second usage is very formal and not so common, although you do see it quite a lot in legal documents.
