[General] Word-“Sweetheart”, usage and other meanings

What is the common rule to use the word “sweetheart” to someone?
If someone addressed us “sweetheart”, what does it mean? And how it can be addressed back while giving the reply?

Use this:
If you are addressing your romantic partner.
If you are addressing your child.

Possibly if you are a woman addressing a small child not your own.
 
In some dialects in the UK, it can be used to sound friendly when speaking to someone, even strangers. However, I would follow Barb_D's advice.
