Default Address format

Hi,
I want to know how to write address in english exactly. i read some book and websites but it seems that addresses in foreign countries is a little different(at least in addresses i saw). is this type of address correct(for formal letter):

4th floor, No 65, 25 Laleh street, Keshavarz blvd,Tehran, Iran

In the above example, it would probably be best to address the envelope:

Person's Name
No. 64 Laleh Street 4th Floor
Keshavarz Blvd.
Tehran, Iran

Thank you Dear Ouisch but how about street number(25 Laleh street)?
If i want to fulfill a formal form or application how should i full address field?

Yes, the 25 should be included in front of 'Laleh Street.' That was my error, sorry.
