Default Writing a letter to a dame

I need to write a letter to a Dame, Could you tell me how I should refer to her? Should I put dear Dame or Dear Dame Anne? Either way looks wrong at the moment.

"Dame" is the female equivalent of "Sir" in the honours system, so:

Dear Sir Alan

Dear Dame Emma
