Letter to file a FIR for lost mobile phone

I've lost my vodofone sim and mobile, Please provide me a format to write a letter to file a FIR so that I can block the sim and get a new sim with the same number.

How you lost your mobile phone?
Here is one draft to file a FIR

Nokia Mobile 6610, Mobile No. 988******* is stolen at 9:00 AM on dated 27/06/08 in Bangalore. I was going to office by Bus route no. 75 and got pick pocket my mobile at Indiranagar, Bangalore.

For blocking of sim you have to write to Vodofone office.
