Vacation Request for Medical Treatment

Hello everyone,

I would like to write a letter for medical vacation request but am a bit perplexed about how to word it.

Dear Mrs. Smith,

This is my formal letter requesting my vacation time. I would like to combine my vacation time from last year and take a months vacation from June to July. I plan on taking a trip to my home country of Romania because near my home town we have natural spring spas. As you are well aware of my injury with my sciatic nerve, I would like to try this holistic approach, as recommended by Dr. James Maloy. He believes that it may benefit me greatly; as well it is a necessary step in avoiding permanent damage which will eventually lead to surgery. I hope you understand and grant my request.

I would like to combine my floating vacation days from May 15 – May 31 2008, as well as June 5 – June 24 2009 in order for this therapy to be effective. I hope you understand and grant my request.
