help...how to write leave of absent letter.. urgent

hi.. can someone help me..
i need to write a letter to school explaining my absent..
i dont know hot to write a formal letter.

Informing that I was absent from school on my third and fourth term, because at that time I found out that I have some medical problems and as for that has to undergo some treatments and therapy to improve my health condition both in KL and Singapore.
The therapy ended one week after my fourth term has started. So it is too late for me to join that term and I must wait until the term ended and join the next term.
