The first two are correct. You can tell more easily by expressing them as "This belongs to you". -> "This does belong to you". -> "Does this belong to you?"
You might be able to recognise that "This is belong/s to you" is not correct, so neither is the question, "Is this belong/s to you?".
Whether you use 'this' or 'it' depends on other factors, as you know.
