getting a neck

Hi folks,
The Golden Girls Season 3 Episode 4 starts with the following conversation between Dorothy and Sophia.

Dorothy: Have a wonderful time at the wedding. Be sure to give Phil and the family my love.
Sophia: Before I go, I need your opinion on something.
Dorothy: Yeah?
Sophia: What should I give Phil's daughter as a wedding gift? Your grandmother's lace tablecloth, or 50 bucks towards getting a neck?

Could anybody tell me what Sophia means by “getting a neck?”

Is this person perhaps of the physical appearance that she has no neck? Some larger people can seem that way.
