He got injured -- this refers to a specific incident. A happening or an incident. (Hindi - Aise hua)

He was injured -- this refers to a specific situation. A specific circumstance, or the way things were at a particular time. (Hindi - Aisa thha)

This is ambiguous. It could be read that injured is an adjective describing his state during play. It could also be that injured is a verb in the passive voice, paraphraseable as 'Someone injured him while (he was) playing football'.

I think most people would take it to mean "He got injured while playing football" rather than that he played while injured.

He could either have injured himself or have been injured by someone else.

I don't think so. To describe that situation we might say, "He played while injured" or "He played despite being injured" or "He played despite an injury".

He could either have injured himself or have been injured by someone else.
He might have been injured without the involvement of anyone else, but I wouldn't call that injuring yourself.

I'm not sure I've followed. If I'm playing football, and stumble and fall while running for the ball and twist my ankle, I might tell someone who asks me why I'm limping, "I injured/hurt myself playing football".
