He said about his "friend"? / "friend?"

I have learned that we put question and exclamation marks inside quotes if the sentence in them is a questions or an exclamation, and I have also learned that we put them outside quotes if the sentence in the quotes is not a question but the main sentence is one. I also know that, if both sentences are questions or exclamative, we put the right mark - the question or the exclamation one - inside the quotes.
However, I still have two questions. Help me sort them out, please.
1. Do we put exclamation and question marks inside or outside quotes if the information in the quotes is not a direct-speech sentence?
E.g.: 1) What did he say about his "friend"? / "friend?"; 2) I hate your "parties"! / "parties!"
2. Also, if one sentence is an exclamation and the other is a question, do you put two different marks or only some particular mark?
E.g.: Did he say: "I hate you!"? / Did he say, "I hate you!"
 
Did he say "I hate you!"? is correct.

The quotation marks should go around only the words and punctuation that were part of the spoken sentence.
 
1. What did he say about his 'friend'?

2. Did he say "I hate you!"?

Note that 'friend' is probably not meant as quoted speech.
