I'm a bit puzzled by the verb "head" when used with an object pronoun. To head is to hit a ball with your head. So here it means to hit the ball with our head. Does it make any sense? But I guess to "head" here means to put or make, so if I were to paraphrase the sentence, it might read "he puts us level."

The verb heads here means 'scores with a headed goal'.

Yes, you could paraphrase it to 'he puts us level by means of a headed goal'.
