To be extremely nervous, alarmed, scared.
 
That means ; whatever s/he feels is always uttered by her/his mouth.
To put it simpler, children say what they see and feel at the moment. The old, too :)

This is an interesting interpretation, however the idiom itself has been in use for over 400 years and means something quite different. In addition to my earlier suggestion, you can check "Answers.com."
