[Grammar] 'Something' in questions

This is a sentence from a manual on writing dialogues by James Scott Bell:

"Rach, truly, is something wrong?"

Would it be different if I used 'anything' here?

The short answer is yes. When you use a different word you almost always change the meaning.

You are likely to get a different response with a different question.

With no context, it's not possible to say what the difference is.

Why don't you create a scenario where somebody asks Rach that question? (You can leave out 'truly', which sounds odd there.)
