Is number one correct only?
1) He was killed with a knife.
2) He was killed by a knife.

Yes, though you will hear/see it. He was killed by the person who used the knife.

But you changed the position of "only" from the end of the sentence to the beginning. Do you mean it is wrong to put it at the end? I saw some examples with "only" at the end of the sentence? How do you justify this?

In most dialects "only" at the end is wrong, a notable exception being Indian English.
