Kid of the mum: Come on, Jack (the kid's name). He have had told you not to do that.

Is he have had or he had?
 
There's no such thing as "have had told". "He had told" is wrong in this case.
"He told you not to do that." - Simple past. There's no indication for the past perfect (had told) here.
 
Yes, there are other grammatical errors. I only commented on the one asked about.
No, you'd use 'forgot' with 'took'. "He was forgotten" is wrong here.
You could probably say "He has already told you..."

If the second speaker (Kid) is called Jack, why not just label speaker two as Jack?

You now know that "he have had" is grammatically incorrect. It looks like you need to study the construction and usage of various past tenses.
