Note my corrections to your question (which should have appeared on one line, not two).Which sentenceswilldo/would you consider to be correctly formulated passive constructions?

Thank you!
I think sentences 1, 3, and 5 are in the passive voice, but according to the book's answer key, sentence 4 is included as well.
Thank you!I don't think one should think of sentence 4 as being in the passive voice. It's better to think of 'performed' as an adjective since it's being modified by 'most'.
Semantically though, yes, there is a passive sense here in that 'Hamlet' is playing the semantic role of patient. A close paraphrase would be: 'Hamlet' has been performed more than almost any other work in literary history—clearly in the passive voice.
Thank you! Here's a funny little test that I was taught to apply to sentences to see if they are/could be passive. See if you can add "by zombies" to the verb part. If you end up with a grammatically possible sentence, it's passive.
Sentence 4 would become "Hamlet is one of the most performed works by zombies [in history]". Ridiculous though it is, it's grammatically possible.
