Well! Here "going to" can have two meanings:

1) It includes the whole process of joining the university (putting in his application, sitting in entrance examination etc)

2) Literally going to the University.

Examples:

1) After his A Levels he went to the University.

2) He goes to the University at 7 o'clock in the morning.

In the question asked I think it has the second meaning but with a difference, i.e. He has many things to do/ responsibilities to take care of BUT (if he gets through the entrance examination (which here is really difficult)) he would prefer going to the university among other things that fall to him to do.

Now the sentence in question:

(Among many other responsibilities that he has to take care of) Going to university would be his first choice but it is difficult to pass the entrance examination.

When you say first choice, it implies there is a second choice, doesn't it?
I would prefer 'priority'.

The second clause seems unrelated to the first. It should be changed to:
...he thinks it is difficult to pass the entrance exam.
